Although the fatal infection caused by Neisseria meningitidis is not uncommon, Neisseria perflava is rarely the infecting organism and has not been reported in Korea. A 36-year old male driver was brought to Yonsei. Medical Center in semicoma. He had fever and generalized petechiae. Repeated blood cultures grew N. perflava and the titer of antibody in serum was 1:640, whereas that of the control was 1:20. He failed to various antibiotic therapies and expired on 38th hospital day. Autopsy showed acute meningitis, subacute bacterial endocarditis and multiple mycotic aneurysms of the cerebral vessels and the right popliteal artery. There was vegetation and perforation of the aortic valve and subarachnoidal and intracerebral hemorhage.
